The Coordinator Services is delivered by a Transition Coordination Agency (TCA). The purpose of this service is to help an individual relocate to a community-based setting upon discharge from a long-term care facility.
Community Transition Service (CTS) may also include the provision of items necessary for patients to effectively establish a community-based residence. This includes moving expenses, security deposits, utility deposits, one-time pest eradication service, and items purchased.
